Types of Medical Cannabis Cards
- For patients aged 18 and above.
- Initial validity of six months, followed by another six months upon renewal.
- Patients aged 18-20 require approval by the Compassionate Use Board.
- For parents or guardians of patients under 18.
- Valid for 60 days initially, then six months upon renewal with an additional fee.
- Requires a criminal background check.
- For designated caregivers of active patients aged 21 and above.
- Initial validity of 60 days, followed by six months upon renewal with an additional fee.
- Requires a criminal background check.
- For patients not residing in Utah or those who’ve been residents for less than 45 days.
- Must possess a valid medical cannabis card or its equivalent from another state, district, or territory.
- Issued for up to two visitation periods per year, each lasting up to 21 days.

Application Process
1. Application Process
Begin your journey by identifying a qualified medical provider (QMP) in Utah who is authorized to recommend medical cannabis based on your health condition.
2. Schedule a consultation with the provider either in-person or via video call.
Once you’ve identified a QMP, set up an appointment to discuss your medical condition and determine if medical cannabis is a suitable treatment option for you.
3. Create a Utah ID account online.
Before applying for the card, you’ll need a Utah ID account. This online account ensures that all your application details are securely stored and easily accessible.
4. Apply for the medical cannabis card online using the EVS Patient Application Guide.
Utilize the Electronic Verification System (EVS) guide to navigate the online application process smoothly, ensuring you provide all necessary details.
5. After profile creation, the medical provider will enter a certification and recommendation.
Post your profile setup, your QMP will certify your eligibility for medical cannabis and provide a recommendation tailored to your medical needs.
6. Pay the $15 medical cannabis card application fee online.
There’s a nominal fee associated with the application. Ensure you pay this online to proceed with your application.
7. In most cases, a temporary card is issued for immediate use. The DHHS will then issue the official card and email a copy to the applicant.
While your official card is in the works, a provisional card is often provided so you can start your medical cannabis treatment without delay.
8. To purchase medical cannabis, present the medical cannabis card and a valid photo ID at a medical cannabis pharmacy. A consultation with a pharmacist might be necessary.
